CERTIFICATION/LICENSE AND LIABILITY INSURANCE
Please note that to rent our studio, you must have the appropriate certification and/or license for the modality you wish to offer. We also require professional liability insurance and a signed waiver from each of your students before they can practice in our space.

• Please describe (in detail) the type of classes or services you would like to offer and the estimated number of participants.

• The date(s)/time(s) that you would like to rent the space, and whether you are interested in a one-time or weekly rental. Please check our class schedule and workshop schedule first to see if the days/times you desire are available.

• List any equipment you would be bringing into the studio.

• Let us know how much set-up and take-down time will you need.

• Tell us a little bit about yourself and include a link to your website or social media.

• Confirm that you have the proper certification and/or license and that you currently have a professional liability insurance policy.
